KFRC in Vallejo offers SCI Wellness Program for KP Health Plan members

Kaiser Permanente Health Plan members with a spinal cord injury are now eligible for a SCI Wellness Program offered by the Kaiser Foundation Rehabilitation Center (KFRC) in Vallejo, a CARF-accredited SCI specialty center which has served people with SCI since 1948. This program aims to support all aspects of healthy living post-SCI and is made up of a treatment team that includes a rehabilitation doctor, physical therapist, occupational therapist, rehabilitation nurse/case manager, and an equipment specialist who have expertise in caring for people living with SCI. During the clinic appointment, the team will complete a comprehensive assessment, make referrals for services and specialists, review resources, provide education and recommendations for healthy living, and collaborate with other health providers within Kaiser Permanente.

The clinic team addresses individual goals and reviews topics such as current neurologic status, pressure injuries, aging with a disability, equipment, issues with pain and spasticity, body systems (bowel, bladder functioning, sexual health), peer support, recreation, physical activity, and more. To participate, have your Kaiser physician or nurse practitioner send an “e-consult” referral to Vallejo Physical Medicine & Rehabilitation.

Appointments are scheduled on Wednesdays, 2-hour in-person visits, or 1-hour video visits.  Additionally, prior to the visit, there will be a 1-hour phone consultation with a rehabilitation nurse to review health status, current problems, and discuss goals for the clinic visit. The wellness clinic is located on the 3rd floor of the hospital tower, Kaiser Vallejo Medical Center, 975 Sereno Drive in Vallejo.
